Job Description:
A company is looking for a Care Coordinator to support patient
care and management.
Key Responsibilities
Engage high-risk patients and ensure follow-up appointments with
Primary Care Providers are scheduled
Collaborate with care team members and external vendors to
facilitate patient care and support
Request medical records and assist nursing and social work teams in
patient follow-up and monitoring
Required Qualifications and Education
3+ years of experience in a medical office or healthcare
setting
2+ years working as a Licensed Practical Nurse (LPN), Medical
Assistant (MA), Pharmacy Tech, or Community Health Worker (CHW)
preferred
Comfort with technology, including Microsoft Office products
Prior experience using electronic health records for data capture
and reporting
