Care Management Educator (System) - Care Management Education - Orlando Care Management Education seeks to hire Care Management Educator who will embrace our mission to extend the healing ministry of Christ. Work Hours/Shifts: Full Time: Flexible Monday - Friday Job Summary: The Educator - Care Management - System serves the Florida Hospital Care Management (CM) department system-wide. Develops, assesses, plans, implements, and evaluates procedures and processes for the system, offers continuing education, process improvement, and change management. The CM Educator is actively involved in future planning and strategic development for system CM needs, to include policy development, program development, ongoing audits for the system level and campus level, and quality improvement efforts. Works under general supervision and utilize critical thinking skills. The CM Educator adheres to Florida Hospital Corporate Compliance Plan and to all rules and regulations of all applicable local, state, and federal agencies and accrediting bodies. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. Knowledge, Skills, Education, & Experience Required: Excellent communication techniques in oral and written form Computer knowledge including basic computer skills in Windows and Outlook Excellent ability in making presentations to management, staff, and business partners Comfortable in a complex and changing healthcare environment Medical record investigative review knowledge Master's degree in Social Work OR Nursing Degree (RN, BS, BSN or advanced degree) Experience in Case Management, Managed Care, Utilization Management, Acute Care, Long Term Care, Home Health Care, OR Center for Medicare and Medicaid Services Programs Extensive experience with Microsoft Office, including Excel, PowerPoint, and Word Minimum of five years of acute clinical experience Knowledge of specific Florida Hospital computer-based programs (such as Sunport and Cerner) as well as Microsoft Word, Excel, and Outlook (preferred) Familiarity with Center for Medicare and Medicaid Services Programs (preferred) Ability to read, interpret, and strategize future projects, bringing future state to reality (preferred) Master's degree in Nursing Education, or related health care related field (preferred) Experience in education in a healthcare or Care Management setting (preferred) Licensure, Certification, or Registration Required: Current valid State of Florida RN nursing license Licensed Clinical Social Work (LCSW) (preferred) Accredited Case Manager, Certified Case Manager, or Board Certification in Case Management (preferred) Job Responsibilities: Demonstrates through behavior Florida Hospital's Core Values of Integrity, Compassion, Balance, Excellence, Stewardship and Teamwork as outlined in the organization's Performance Excellence Program Develops, produces, and delivers training for CM on an ongoing basis. This includes monthly New Hire CM training, CM Preceptor Program, and on an as needed basis for current employees in the CM Department. Audits new Care Managers during their 90 day probationary period. Presents written and verbal updates to the Care Managers and their Campus Manager and Director. Provides additional education based on results of audit and provides input for the 90 Day Evaluation. Audits 10% of licensed beds for each campus on a monthly basis for the following scorecards: CM Campus, CM System, and AHS-Corporate. Maintains CM website by reviewing and updating content on a regular basis. This includes, but is not limited to, CM flows, regulatory information, and required forms. Reviews and revises CM Policies, SOPs, and Job Descriptions annually, or as needed. Identifies resources in the community and coordinates education opportunities for CM staff including webinars and continuing education. Acts as the SME (Subject Matter Expert) for Care Management for Revenue Cycle related projects. Acts as a liaison between CM leadership, staff members of the Revenue Cycle, other interdisciplinary team members at all applicable levels of leadership, and external partners. Communicates appropriately and effectively with all involved parties.
